Information, Referral & Assistance

  • Provides initial screening and referral to appropriate resources within the Area Agency on Aging or within the community

 

Benefits Counseling

  • Benefits counseling includes assistance with Medicare claims and appeals, Medicare supplement insurance, Social Security, Medicaid, Food Stamps, disability and long term care insurance

  • Provides legal/benefits counseling assistance on matters such as wills / estates, living wills, durable power of attorney for health care and property disputes.  Assistance is also available with entitlement benefits and in obtaining legal counsel for those who cannot afford to hire an attorney.

 

Care Coordination

  • Individual client assessment and plan of care - arranging for appropriate services.  The Care Coordinator also maintains a waiting list of applicants seeking help from program for residential repair or modification - vision, hearing and dental services, depending on availability of funds.

 

Family Caregiver Support Program

  • The Family Caregiver Support Program is designed to provide assistance, support and services to caregivers.  Caregivers need support in everyday activities that surround the long-term care of their loved one(s).  This program can empower the caregiver with education, information, training, coping / hands-on skills, relief and support groups - this way the caregiver can retain greater control over their own personal life.

 

Nursing Home Ombudsman

  • The Texas Ombudsman program advocates the senior's Resident Rights in long-term care facilities such as assisted living and nursing homes.  Certified ombudsmen from the West Central Texas staff as well as 55 other certified volunteer ombudsmen visit over 58 nursing facilities on a regular basis, as well as participate in open hearings, family and residential council meetings, unannounced inspections, complaint investigations and annual surveys.  The program also assists residents and families with nursing home placements, questions regarding nursing home lifestyles, finances, spousal impoverishment and Medicaid / Medicare eligibility.

 

Nutrition Programs and Services

  • Provide home delivered or congregate meals through local service providers (in most areas).

 

Prescription Assistance Program

  • Our Prescription Assistance Program is able to assist clients or their advocates in finding the best options for prescription drug bill elimination or reduction.  Call us for an appointment or complete our request form.
